The role of the Geriatric Care Manager is to advocate and provide support to seniors through coordinating the resources necessary for seniors to remain independent in their homes for as long as possible while not sacrificing their safety and quality of life. Laurie assists seniors and their families with determining when the time has come for a variety of lifestyle changes such as no longer driving, medication management, companionship, or discussing the need for a more supportive environment that may include in-home help, assisted living, or skilled nursing home care. With sensitivity Laurie continues to support seniors and their families as they adapt to new physical, cognitive, or environmental changes.

Care management services are offered in a variety of settings.  Practitioners can offer short term or ongoing assistance for long distance caregivers.  Personalized, gentle services are specifically designed to meet the client’s needs and wants.  Geriatric care managers are often available after hours and on weekends.  Their involvement may ensure continuity of care thereby reducing miscommunications, stress, and cost to the clients.  Ongoing support may enable greater independence and prevent unnecessary moves or hospitalizations.
